My wife and I came back last July and sent off our pension refund forms in the fall. I received my lump-sum payment in December but in early January we got my wife's forms back as there was a mix up with her middle name. We resent the forms in early February and have heard nothing since. I was just wondering if anyone has had similiar experiences and could fill me in on the timeline for a 2nd effort. Or if anyone knows the best way to check on the status (without the greatest Japanese skills) that would be helpful as well.

I wouldn't know which department to call and it hasn't happened to me, but is there a former supervisor or person you trust to whom you designated to receive the tax back from that refund? Perhaps they could ask or find out for you? You also may want to contact the consulate to see if they can steer you to the right department.
